Modest modifications are needed to make this applicable for your company. As always, check with an attorney prior to implementing a new contract or procedure.
Note from submitter: "This was previously reviewed by our attorney upon creation. It is pretty user-friendly. We do not, however, require a personal guarantee on file for all credit card users as indicated; it is a clause used for extra leverage if the customer is desperate to pay with credit cards (frequent flyer miles, etc.). Hey, we will take any additional protection we can get."
